How CDPHP Offers Better Access to Care

Being a member for more than 18 years, Brook Greco of Latham, NY knows firsthand how CDPHP® makes health care easier and more accessible for its members.

Founded and guided by Capital Region physicians, CDPHP works closely with local doctors, hospitals, and pharmacists to create better access and help members, like Brook, navigate the complex world of health care.

Better Access to Doctors

As a wife, a mom of two, and a full-time music teacher who also directs the school musical and select chorus after school, Brook’s days are full. Her busy schedule leaves little time for squeezing in things like doctor appointments.

So when CDPHP partnered with Albany ENT & Allergy Services and three other independent medical practices to open the 1785 Specialty Medical Practices Building in Clifton Park — a world-class medical facility focused on best-in-class care and an exceptional patient experience — Brook was thrilled.

“I’ve seen Dr. Nora Perkins at Albany ENT for years, and I used to have to drive more than 40 minutes each way to see her in Gansevoort due to my schedule.”

1785, which opened in 2021, now provides Brook convenient access to Dr. Perkins, reducing her drive by more than half.

“1785 is so welcoming, says Brook. “It’s easy to park. The central patient registration simplifies things. It’s not hectic. It’s a nice experience for when you have to go to the doctor.”

“It’s just relaxing.”

Better Access to Medication

Most recently, CDPHP teamed up with Community Care Physicians, the largest, independent multispecialty medical group in the Capital Region, to open a state-of-the-art medical building in Latham, called Wellness Way.

The 263,000 square-foot building offers patients a better health care experience with coordinated care from primary care providers, specialists, urgent care, imaging, lab, pharmacy, and health plan all under one roof.  

“It works for me because I live here,” says Brook.

“Our previous pharmacy couldn’t get the medication my son takes. ConnectRx pharmacy at Wellness Way has the medication he needs.”

The pharmacy, which is powered by CDPHP, offers personalized pharmacy services designed to meet patients’ individual needs.

Brook’s experience with ConnectRx has been so positive, she plans to transfer her own prescriptions and take advantage of their free home delivery next.
