Healthy with Your Annual Wellness Visit
Did you know that as a CDPHP® Medicare Advantage member, you can receive both an
annual physical exam and an Annual Wellness Visit at no additional cost?
Both are important because they have different purposes.
- An annual physical exam is where your provider performs a thorough check of your body’s physical
health. This is the type of annual physical you’ve likely had throughout your life. - An Annual Wellness Visit assesses your overall health and well-being. It’s the extra time you need
with your provider to create or update a personalized plan for your health care future.
During these visits, your provider may:

| Assess your risk of falling and give you tips for fall prevention | ✔ | |
| Look for signs of memory loss or depression | ✔ | |
