With so much attention on the rising costs of health care, CDPHP® has found a way to save you money while improving the quality of care you receive. 

It all starts with our Enhanced Primary Care program, which rewards doctors for providing better care, not more care.

This allows doctors to meet your unique needs and gives doctors the freedom to practice in ways that are best for you.

As a result, you get more one-on-one time with your primary care doctor, experience better coordination of care, and have higher-quality health outcomes.

CDPHP is continuing its efforts to expand the program for 2018. Businesses may choose to offer their employees our $0 Copay Enhanced Primary Care plan, which will enable employees to pay nothing when visiting a participating practice.

CDPHP Medicare Choices members can benefit, too. Beginning January 1, 2018, Medicare members will receive a copay reduction of $5 or $10 on most plans when visiting a participating practice – meaning some members will have no copay at all! Check your Annual Notice of Change/Evidence of Coverage to see how this benefit impacts your plan.

Care that revolves around you

At Enhanced Primary Care practices you have more than just a doctor in your corner. The entire practice functions as your “medical home” – a place where doctors, nurse practitioners, administrative staff, and more work as a team to provide the comprehensive care and support you need to stay healthy.

It’s a model of care that truly revolves around you, as health care should.

CDPHP works with doctors at participating practices to implement new ways of serving patients better. Here’s what that means for you:

A personal doctor and care team. You will have an ongoing relationship with a primary care doctor and his or her team.

Added convenience. Expanded hours, same-day appointments, and consultation via text or email may be options. Participating practices have the flexibility to develop a care plan that works best for you.

Care coordination. Practices have the opportunity to embed CDPHP case managers and pharmacists within their practices. These added resources help doctors coordinate patient care and provide comprehensive medication reviews, thus freeing up time for the doctors.

Electronic medical records. Doctors who have access to your medical records are better equipped to provide personalized care. Finding care when you need it.

 Taking the next step

Our Find-A-Doc search tool can help you locate doctors, specialists, and other providers in your area. Look for a [tiny purple house] next to the name of doctors at participating practices. A full list can be viewed by choosing “Enhanced Primary Care” from the “Select a Specialty” dropdown menu.

If your current primary care practice participates in the program, don’t hesitate to ask about it during your next appointment.

 

CDPHP is an HMO and PPO with a Medicare contract. Enrollment in CDPHP Medicare Choices depends on contract renewal.

(CDPHP®) With more than 30 years of experience as a pharmacist and health care industry leader, Eileen Wood plays an integral role in the CDPHP mission of providing greater access to high-quality, affordable health care. Eileen joined CDPHP in 2005 and currently serves as senior vice president of clinical integration and chief pharmacy officer. In this role, she is responsible for the pharmacy benefit, clinical pharmacy programs, health care performance management, and quality management teams. Eileen is also focused on moving the company’s Enhanced Primary Care initiative, a patient-centered medical home model, toward achieving the Triple Aim and delivering health care value to stakeholders. Through strategic partnerships with practitioners and the delivery of actionable information for patient populations, CDPHP seeks meaningful improvements in the total cost of care and health care quality indicators. Prior to joining CDPHP, Eileen served as vice president of clinical services and director of pharmacy services for Seton Health System in Troy, NY, part of the Ascension Health System. Eileen began her career in an independent community pharmacy practice, and her experience includes progressive hospital pharmacy and management experience in oncology, surgery, laboratory, cardio-pulmonary, medical imaging, and other clinical services management. Eileen earned a Master of Business Administration from Rensselaer Polytechnic Institute in Troy, N.Y., and a Bachelor of Science degree in pharmacy from St. John’s University in Jamaica, N.Y. She is licensed as a pharmacist in New York state.
