When Care Goes Beyond Coverage: Paula’s Connection with CDPHP®

“I never felt this kind of support until I became a CDPHP® member.”

That’s how Paula, a CDPHP Medicare member from Schenectady, describes the deep connection she’s found with her health plan.

In the fall of 2024, Paula’s husband moved into a nursing home. Living in a rural part of Schenectady County and unable to drive, Paula was overwhelmed.

“Admittedly, I panicked,” she said. “Uber rides and taxis were going to be so expensive – about $25 to $35 each way. I couldn’t afford that. My wonderful kids help on weekends, but I wanted to visit my husband during the week, too.”

That’s when Paula discovered Papa Health, a benefit available with most CDPHP Medicare Advantage plans. Through Papa, carefully trained companions known as Papa Pals began picking Paula up, taking her to visit her husband, and driving her home—at no additional cost.

“I don’t know what I would have done without Papa,” Paula said. “It’s a wonderful program. Just knowing someone was coming to get me made me feel less alone.”

When Paula’s available hours with Papa were nearing their end, she worried again about how she’d continue her visits. Fortunately, her CDPHP case manager, Kat, stepped in and connected her with Community Caregivers, a local nonprofit network of dedicated volunteers whose mission is to help individuals maintain independence and dignity at home.

“I’m grateful to be able to connect CDPHP members like Paula with Community Caregivers for the extra support they need. Paula was my first referral to the program, and now she can continue to stay close to her husband of more than 50 years. Their separation when he entered a nursing home was very difficult, but through consistent weekly visits, they’re able to maintain their close bond,” said Kat, social work outpatient care manager at CDPHP.

“They were there for me when I needed them most,” Paula said. “When I found out they could help, I cried. Seeing a smiling face come around the corner to pick me up—it makes my whole day.”

Now 81, Paula is managing her own health challenges while also caring for her husband. She relies on a strong network of volunteers, family, and CDPHP-supported services to stay independent and connected.

“Volunteers don’t get enough credit,” she said. “I couldn’t live in my home without them. Everyone from CDPHP and the organizations they’ve connected me with has been so kind. God answered my prayers.”
